I got it from one of Stella's Scholastic Reader order forms. I have also seen similar ones at the dollar store.

Time for something new. By 10:30 a.m. Stella had filled out her own reward chart for the rest of Saturday and as you can see, she already decided that she wasn't going to LISTEN & OBEY or CLEAN UP.
